We fired some more and she noticed the slide kept locking, I said "yeah it does that every time!", and turned the gun on its side. I said "Oh! Wait a minute, I think I figured it out!" Seems there is a little spring which slips into a groove on the slide pin which wasn't in the groove. It is barely noticeable and wasn't something mentioned in the manual. I pulled the slide pin out and reseated it. Final test and Viola! It fired perfectly! Every recommended ammo type we had bought worked flawlessly!
