Wife's application, name change

She took her class at carry legal before we got married but now she has a different last name and hasn't turned in her packet yet. Has a new drivers license and everything with my last name on it. Should she just bring the marriage license to the court house? Tried to call carry legal but no answer.

Nvm, failed to understand the question. Yes there is a box that she can enter her maiden name on the application.

I changed my name back in 1978,,,
All I did was copy all of the documentation and send it in with the application packet.

There were no problems at all,,,
CWL was at the Sheriff's office in 49 days.
