The rules are simple.

There are 5 scenarios. I'll post 1 scenario each day in it's own thread. Some time between 24-48 hours later I will post the answer as provided by the article.

Read the scenario and then explain how it would be possible for this to happen.

First of all pulling the trigger to make sure a loaded firearm is safe is a dumb idea.

My guess is that when he cycled a round into the chamber he short stroked the lever, which caused poor engagement of the hammer. When putting the rifle into half cock mode, the hammer was just barely hanging on, and was shook free by the minuscule amount of play when thumbing the trigger like a tard.
