For those nosy doctors that quiz your kids to find out if you have guns in the house.
While I don't recall ever being asked about firearms by a doctor, I have read on several forums that others have been.
It would be fun to see the look on the doctor's face when you hand him one of these to sign. Of course you may want to look for a different doctor after that.

We were asked when my son was about 7 & he answered truthfully, HE SAID "I have my own, bear cat, 20 ga, 22hornet & 30-30 handi rifle" Dr said cool & that was that
He also did an essay in 7th grade about the 1 posession you would keep. His answer "M1 Garand", Teacher in front of the class tried to make owning a gun a bad thing as he put it. His answer "So I can protect & feed my family"
This set off a big debate in class, most of his friends have shot with us & piled it on. We got a call from the teacher & She tried to put all this in a negative light until I said Hold on He did the right thing conversation over.
Got to love the way an 11 year old boy thinks. Just that summer we took a trip to the CMP & pick the M1 up so it was fresh in his mind & his favorite gun at the time, probablly still is wants to shoot it every time we go out

To me, this opens the door for down the road having to have my license and my Dr's Statement that I am competent to own a firearm. I personally do not care to get something like this started.
I like it. I'll get my doctor to sign next time he tries to mooch shells from me so he can beat my a$$ shooting trap.
Stupid form. No doctor in their right mind would sign that, but I'll bet they would be more than willing to have you take your medical needs elsewhere.
