Not a actual squib, but a light load once. Factory ammo. 41 colt cartridge.
Fortunately it was heard by the shooter and those of us close to him.
Sent the bullet about 1 inch into the barrel. We were able to punch it out on the range with a pencil.

I was spotting at a CAS match one day, a lady's .45Colt Vaquero went "Pffttt!" I yelled at her to stop, but the TO did actually see the bullet come out so they allowed her to continue. Her next round went "BOOOOM!", she nearly brained herself from the recoil. I told her "I know where the powder that should've been in the first round ended up." Just glad she was shooting a Ruger. A Colt or clone probably would have blown up.

Speaking of semi-auto, I blew up a 1911 barrel. Gunsmith said I had to have had a stuck bullet, and the next one blew it out and split the barrel. So, if that's what actually happened, and I still don't know it for a fact, a squib can cycle the action of a 1911.
