Also looks like a burr at 4 o'clock. A 1/8 wood dowel rod wrapped in fine emory cloth chucked up in a drill does wonders. Tape end of emory cloth or cut a slot length wise down the middle of dowel rod length of emory cloth width. Have cleaned up a many of chambers with issues using this method.

Had a AR10 .308 chamber awhile back that you thought they hammered it out with a punch. It was a friends that had no spare change to replace. Every shot the case would stick in chamber. I polished it out starting with 220 grit working up to 1200 grit. When I was done chamber still measured in spec and problem alleviated.

I agree that appears firing pin damage at 12, and especially so if slide has firing pin at same location...
And ironing or knocking off the burr might fix it...
But, my question is, why is it not doing this when manually cycled? Only when fired?
Simple answer. Case expansion.

When fired, the case expands and will then grab onto the burr(s).

A chamber iron (Brownells still sells those), pushes the metal out of the way....but if the burr is minor, I have dealt with them simply by wrapping a small bit of dowel or small round Swiss file (being careful to not score far side of chamber with file tip) with emery cloth, we are not talking a 65kpsi cartridge, here, and if not enough material removed to allow expansion/rupture, then no harm/no foul.
