Excellent point. He is indeed lucky that all he lost was a gun.
My understanding is: he was using an RCBS scale and weighing each charge, not using a measure or dispenser. Somehow the poise (I been trying to remember that word for about a week now, finally came to me.) got bumped up to the next setting and he didn't notice it. He SHOULD have noticed he was pouring a bunch more powder than normal from his pan into the case. Like I said, complacency is your enemy.
On a sort of related topic, not all electronic scales are suitable for trickle charges. I have a Cabela's scale that will not change readings if you trickle extra powder in very slowly. I can't remember the numbers right off, but let's just say I wanted 40 grains. I would put around 35.5 grains in with a dipper and then trickle the rest. As I was trickling I kept thinking "Why isn't this thing changing?" Finally I lifted the pan of the scale and set it back down and had 40.5 grains. I do all trickle charges on my 505 now. I posted this a couple of years ago, and someone explained something about the auto zero built into the scale is what kept it holding on 35.5 grains. Not all electronic scales do this, but mine does.
