Well, there is another side of the story now, it was not a prop gun and was not during filming a music video. Sometimes I really wonder how many "accidents" are really accidents?

“He was at his apartment getting ready to go work and do a video shoot,” said Atherton. “We don’t know how, but the gun went off; the bullet entered his head. It was ruled an accidental shooting by the Montgomery County Sherriff’s Department in Texas.”

If he was shot in the head, It wasn't a prop gun. Just my opinion. I know Bruce Lee was killed by a prop gun on set, but this is different. From what I've read, he was an up and coming Country star. Probly uses real guns on his music video cause they couldn't afford a 'stunt gun."

I've since read somewhere that it was NOT a prop gun, and in fact, he was preparing to go to work when it happened. If I wasn't getting ready to head back to Oklahoma today (been in Kansas visiting family), I'd search and find the story I read.

Colt (or maybe S&W) used to have a print ad that warned you to keep an empty chamber under the hammer for just this reason. In particular, a lot of old revolvers (and semi-autos) aren't drop-safe. That's why safeties like firing pin blocks and transfer bars exist today.

Odds are that it was the result of a mistake, but you can't eliminate mechanical failure based on the facts we have.
