What I read was it was a derringer with no trigger guard, carried in his jacket pocket. That had bad idea written all over it.

"A Derringer, small two-shot pistol with no trigger guard. Officers said when it fell out of the 72-year-old man’s jacket in the doctor’s office the gun went off."

I've got one of those Davis derringers with no trigger guard. It takes vise grips to pull that trigger. Plus there is a crossbar safety on it that won't allow the hammer to fall IF the safety is on.

+1 - Most hospitals are not located in the best of areas. You tend to keep your trama centers closely located to where the most trama injuries occur.
