A barrel obstruction. My guess is a squib round that didn't make it out the barrel.

Obstruction. The most common cause is someone leaving cleaning stuff in the barrel - patch, jag, brush, cleaning rod, etc.
Homemade cartridges are probably up there somewhere too.
Whatever caused it, it was in the barrel and not in the suppressor or whatever's on there or it would've probably flown away (suppressor)
