If it is food you prepare at home for yourself and family, it is included. If takeout food or for you pet, it is not included.

I'll be surprised if the stores don't creep their prices up to capture the 4.5% themselves.

Also, a rotisserie chicken is still taxed but a party tray is not? That makes sense. Lots of stuff on the exempt list shouldn't have been on it. Vitamins being one.
