I've stopped a couple of guys from blowing up their rifles with their cleaning rod still in the barrel, and I've seen two very similar to that one that were blown up from forgetting to remove the cleaning rod. Usually happen when guys get chatty at the range. It is almost always a "precision" rifleman and his buddy, but it also plagues the guy who brings like 5-6 rifles to the range at once and likes to clean between shots on each one, they soak while he shoots the others then dries them with a patch just before shooting. Why they don't pull the bolt and clean from the back is beyond my comprehension. Hard to insert the bolt with a cleaning rod in the way!!
