I'm pretty sure that's a factory Savage bolt handle.
Diameter looks a bit bigger than the factory one on my 10FP. Plus it's got some texture on the top of it, not unlike the factory Rem 700 bolt knob. The finish looks different as well. Could just be the pic fooling my eyes, though.
I don't see the texture on the top of it, but looking at it more, it does look larger than the typical ones.

Me? Not sure. I'm not even sure where I want to go with this rifle. Tactical, or just heavy target. Also not sure how much I want to put into this gun, since to go very far on either, it will require a new barrel ($250-350), stock ($250+), trigger ($125+), scope ($400+) and mounts (~$100), plus action work ($?$?). Might as well just buy a blank action to start on. So then I'm left with the question again, what to do with this rifle?

If I were in your position, I'd sell it and get an SPS tactical that's ready to rock. Drop it in the new Choate Sniper stock and it would be a sweet sub-MOA rig on a budget.
My groups between that set-up and the $2,000 re-built custom gun probably wouldn't be all too different. I'm not a bad shot, mind you, but I'm not shooting terrorists at 1,000 yards, either.

This one's not going anywhere, it was a wedding gift from my wife. So I'm thinking maybe lower end aftermarket heavy barrel, refinish the whole thing black or something, and shoot it.

I'm pretty sure the bolt knob is factory. It was that way when he showed it too me right after he bought it new.
