0 cost health insurance commercial

I keep seeing this on TV, be under 55, make less than $50k and not on Medicare. Just call and they check your Zip code and see if you qualify AND get $1400 cash bonus.
What is this nonsense? We all know nothing is free so whats the catch here?

They play this commercial a lot on the Newsmax streaming channel and I think I've seen it on Channel 9 in OKC.
