Just to ad a bit. All chambers vary a bit. Factory brass or rounds are made to fit the smallest chamber. A big goodie for you on using Lapua brass, none better. And treated right they will last for at least 25 reloadings. Maybe need annealing now and then. If this is a gas gun, then you need to full lenght resize each time. Getting a proper die is a problem. If you will take 3 of your 3 times fired cases, unsized and send them to Harrells http://www.harrellsprec.com/ They will make you a full length sizing die custom made for your chamber. Cost is $70 and will be the best $70 you ever spent. About ever 5 or 6 reloads you need to anneal the neck and shoulder. A site with a lot of info. http://www.6mmbr.com/annealing.html. I just wanted to thank everyone for their advice. I tried a little bit of everything and got it figured out. I loaded 50 rounds and tried them out at Banner earlier today with zero problems or failures.

What did you find to be the main problem, or were there multiple solutions?

I think it had to do with neck tension. I think when I was seating the bullet, it was doing something to the shoulder. So I cleaned up the case mouth and readjusted my seating die and it corrected everything.
