I went in the hospital last week and they asked what kind of work I did.
When I said warehouse, they asked if I drove a forklift or any other heavy equipment.
They then said they would send a note to boss explaining I was not to operate equipment for 48 hours
until anesthesia completely wore off.
A liability issue.
Most questions have a legitimate reason for being asked.
You paranoid people need to get back on your meds.

If I saw that question, I would leave it blank. If they reviewed the form and asked be what my answer was, I'd ask, "What does that have to do with my (fill in ailment here) current problem? I wasn't shot, so move on.

If they persisted, I would get up and leave.

Just remember how you answer. Its for when they give you some mind altering drugs. Its to cover their butt in case you commit suicide or go on a shooting spree. Also its a form for a thousnd different people entering the hospital for a thousand different reasons. Leave it blank and see if they demand you answer it. I doubt it.

So what are they going to do if you say you have them and they aren't locked up? Not give you treatment because of what you might do when you leave?

Demographics...and I don't answer any of them...they pass the info off to somebody else and like others say, "It's not relevant to why I'm there." I like it when they hand me the papers and say you have to fill these out...I take them and leave them in my chair. I don't have to fill out anything other than my insurance information and how I'm going to pay for services rendered. But thanks for playing.

Just skip it and move on.

I wouldnt answer it. Its none of their damn business.

The answer to the question is a simple- 'N/A' or 'Not Applicable'. Or just leave it blank.

I simply would answer "no".

Here's why.

Doctor prescribes your scitzophrenic ass Xanax, 6months later you get a notice from the OSBI, that your SDA has been revoked.

Make sense?

Well get use to it, it is normal business.
